| | A survey system I threw together one day. It supports multiple choice "R"adio buttons, or "C"heckboxes. move around the paths in the conf files, and in the .shtml file to the proper directories. Also, make sure that the survey data files are writable by whatever user your web server runs as.

| | Simply install Master Poll Taker (directions are right in the script) and set up a form with radio buttons next to the possible answers. The user selects one of those and then clicks to submit. Each possible poll answer can be emailed to a different address. If you have a limited number of email addresses, you can have different subject lines depending on the answer.
